Care Coordination Specialist
Job Summary
We are seeking a compassionate and proactive Care Coordination Specialist to join our team. As a key member of our care coordination unit, you will play a pivotal role in planning, coordinating, and monitoring care and support services for clients.
Working closely with clients, families, and support staff, you will ensure that services are delivered with compassion, professionalism, and efficiency.
About the RoleAs a Home Care Coordinator, your day-to-day activities will include:
- Managing incoming referrals and conducting client assessments to identify their needs and goals.
- Developing individualised care plans tailored to meet the unique needs and requirements of each client.
- Coordinating staff schedules and matching carers with clients to ensure seamless service delivery.
- Monitoring service delivery to maintain quality and client satisfaction.
- Maintaining accurate client records and complying with relevant policies and procedures.
- Responding promptly to incidents, feedback, or concerns.
To be successful in this role, you will need:
- A minimum Certificate III in Aged Care, Individual Support or equivalent (essential).
- A current First Aid Certificate.
- A valid Australian Driver's Licence and access to a reliable vehicle.
- A current Police Check.
- An NDIS Worker Screening Check.
- Experience working with home care packages.
- Excellent interpersonal skills, with the ability to build strong relationships with clients, families, and staff.
- A sound understanding of the Aged Care Quality Standards and person-centred care principles.
- Confidence using care management systems and Microsoft Office.
