Job Title: Care Partner

As a Care Partner, you will work collaboratively to deliver high-quality patient care under the supervision of a Registered Nurse and within the scope of your designated role.

  • Provide personal care services to patients including bathing, grooming, toileting, and skin care.
  • Assist with hair washing, shaving, mouth and denture care, eye and ear care, and support with daily living activities.
  • Contribute to collecting vital signs and other relevant data as directed by a Registered Nurse.
  • Report changes in patient condition or responses to care interventions to the Registered Nurse.

Additionally, you will engage with patients to understand their needs, concerns, and preferences, and communicate these findings to the healthcare team.

Effective collaboration and clear communication are essential skills for this role, enabling you to contribute positively to the delivery of patient-centered care.
