Community Care Navigator

We are seeking a skilled and compassionate Community Care Coordinator to deliver high-quality nursing care in the community setting.
The ideal candidate will have excellent communication skills, be able to work independently and as part of a team, and demonstrate a commitment to delivering person-centered patient care.
Key Responsibilities:- Coordinate and deliver high-quality, person-centered nursing care in the community setting.
- Collaborate with clients, their carers, General Practitioners and other service providers to develop and implement care plans that meet client needs.
- A current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Demonstrated commitment to delivering high-quality patient care.
