Community Care Nurse

We are seeking a Registered Nurse to join our healthcare community, caring for the people of Northern NSW.
The ideal candidate will have excellent communication skills and be able to work effectively with clients, their carers, General Practitioners and other service providers to develop and implement care plans that meet client needs.
Key Responsibilities- Care for patients in their homes, providing high-quality nursing care and collaborating with healthcare teams to ensure seamless care delivery.
- Develop and implement individualized care plans, working closely with clients, their families, and healthcare professionals to achieve optimal health outcomes.
This role requires a strong commitment to delivering person-centred care, empathy and understanding towards patients and their families, as well as excellent organisational and time management skills.
