Patient Flow And Access Care Lead

We are seeking an experienced Nurse Manager to lead the coordination of patient flow and access across our Remote Health Service. In this role, you will be responsible for ensuring that patients receive the right care in the right place at the right time.
To succeed in this position, you must have a strong understanding of patient flow and discharge planning principles, systems, and processes. You should also possess proven clinical leadership skills and be able to lead a team with a commitment to person-centred leadership.
Key Responsibilities- Lead the coordination of patient flow and access across the Remote Health Service
- Ensure that patients receive the right care in the right place at the right time
- Develop and implement strategies to improve patient flow and reduce wait times
- Current nursing/midwifery registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Demonstrated understanding of principles, systems, and processes related to patient flow and discharge planning
- Proven clinical leadership skills and ability to lead a team with a commitment to person-centred leadership
