Job Description

As a dedicated caregiver, you will have the opportunity to make a genuine impact within your local community by delivering professional in-home nursing care.

This role involves providing efficient and effective delivery of professional nursing care and support to customers in their own homes.

You will work independently with a wide variety of customers, ensuring that all services are provided in a caring and respectful way.

Key Responsibilities
  1. Providing individualised and personalised support to customers in accordance with their support plans and/or medical practitioner's directions.
  2. Working under the guidance and supervision of medical practitioners, working with allied health professionals, monitoring and communicating the customer's condition, and other health and wellness-related activities.
  3. Demonstrating a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with policy and procedures, and legislative requirements.
  4. Medication Administration and Complex Clinical Care i.e. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and more.
  5. Monitoring and communicating on the customer's wellbeing and reporting on progress of customers.
  6. Providing training, coaching, and guidance to Support Staff/Case Managers as required.
  7. Conducting skills assessments of Community Support Professionals performing complex care services as required.
  8. Participating in staff meetings and attending training opportunities when required.
