Nurse Professional

About the Role:

We are seeking a skilled nurse professional to provide community care services. The successful candidate will travel to clients' homes and deliver comprehensive nursing assessments and case management, applying clinical knowledge to implement planned nursing care.

Key Responsibilities:
  • Conduct thorough client assessments to identify current needs and requirements.
  • Develop holistic care plans from assessments that align with client goals and objectives.
  • Participate in documentation processes as required under quality standards and relevant legislation.
  • Foster effective communication with clients and key stakeholders to ensure ongoing development and review of care delivery.
  • Regularly review each client's progress against their care plan and make adjustments as needed due to changes in care needs.
  • Collaborate with healthcare colleagues and other service providers involved in client care planning.
  • Prioritize safety and competence in delivering care.
  • Maintain up-to-date knowledge and skills by utilizing best practice resources and evidence-based practices.
  • Document provided care according to organizational guidelines and client management systems.
  • Oversee and monitor the delivery of care by support staff and delegate tasks appropriately.
  • Administer medications in accordance with relevant laws and medical practitioner orders.
Requirements:
  • Current Police Certificate
  • Current Driver's License and reliable vehicle
  • CPR Certification
  • Degree in Nursing or equivalent qualifications and experience at Registered Nurse level
  • AHPRA Registration and evidence of recent practice
  • Experience working as a Registered Nurse in community aged care
