About the Role

As a Support Coordinator, you will work closely with NDIS participants and their families/guardians to connect them with services and supports outlined in their NDIS Plan.

This is a full-time role located in Allambie Heights.

The main responsibilities include:

  • Consulting with clients and their families to identify their needs and funding
  • Coordinating services and supports, liaising with government agencies, obtaining quotes and organising service agreements
  • Developing, monitoring, reviewing and reporting on client NDIS plans and sharing progress with key stakeholders
  • Working within allocated resources and regularly monitoring and tracking billable coordination hours
  • Buidling and maintaining relationships with clients, their families, staff, peers, other services and stakeholders

Requirements

To be successful in this role, you will need:

  • A strong understanding of the NDIS and its funding protocols, standards and processes
  • Qualifications in Social Work, Allied Health, Case Management or equivalent experience
  • Highly developed communication, negotiation and problem-solving skills
  • Strong interpersonal and relationship-building skills with internal and external stakeholders
  • A current Australian driver's licence and car
  • An NSW Working With Children Check, or willingness to obtain
