Clinical Care Professional

We are seeking a dedicated and compassionate individual to join our team as a Clinical Care Coordinator at the Footscray Residential Aged Care Facility.
This role requires an experienced Registered Nurse who can provide high-quality clinical care to residents with complex needs.
Key Responsibilities:- Undertake or supervise others to undertake, clinical care for individual residents as required, including complex wound care, pain management, behaviour management, end-of-life care and other clinical procedures as necessary.
- Liaise with relevant residential care staff, medical, allied health, hospital and other specialist health practitioners to ensure residents receive high level clinical care.
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA)
- Recent experience in caring for high care and dementia residents
- Knowledge of wound care, palliative care, behaviour management, clinical assessments, care plans
