Clinical Care Coordinator Opportunity

Infinite Aged Care is seeking an experienced Clinical Care Coordinator to join our Knoxfield team in Victoria. This full-time role offers a rewarding and fast-paced opportunity to provide high-quality clinical care to our residents.

About the Role

This position reports to the General Manager and provides support and guidance from the Clinical Care Manager. The key purpose of this role is to ensure that all residents receive the highest quality of clinical care.

Responsibilities
  • Provide clinical expertise and leadership to clinical and care service teams.
  • Assist the Clinical Care Manager in leading carer teams to meet all clinical needs of residents, including monitoring and tracking clinical indicators.
  • Manage assessments, documentation, and submission of AN-ACC requirements within the facility.
  • Support effective documentation and user support of policies, procedures, and IT system requirements.
  • Coordinate assessment, care planning, and review processes within the facility.
  • Support the admission of new residents and initial consultations with residents and families.
  • E nsure accurate information is recorded and in accordance with organisational and legislative requirements.
  • Input and oversight into facility team rosters and ensuring optimal skill mix available for delivery of care.
  • Manage and document clinical risk within the facility and ensure a commitment to continuous improvements.
Requirements
  • An innate empathy for our residents and a commitment to continually improving the quality of clinical care provided.
  • A minimum 1-year experience as a Care Coordinator or similar leadership position.
  • A current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
  • Postgraduate qualifications in aged care would be advantageous but not essential.
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
  • An engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team.
  • A high level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
  • A confident team player with excellent communication and interpersonal skills and a passion for leading clinical outcomes.
  • Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations.
  • A valid NDIS Workers Screening Clearance (or ability to apply).
