Job Opportunity: Holistic Care Coordinator

We are seeking a highly skilled and compassionate healthcare professional to join our team as a Holistic Care Coordinator. This role will involve working closely with patients, families, and healthcare teams to provide comprehensive care coordination services.

The successful candidate will have advanced clinical skills in chronic disease management and experience across diverse clinical settings, including general practice. They will be committed to the social model of health and inclusive care, with strong communication and stakeholder engagement skills.

Key Responsibilities:
  • Conduct comprehensive health assessments and identify priority issues
  • Support patients to set goals and develop care plans
  • Initiate referrals to health professionals and services
  • Participate in clinical supervision and stakeholder engagement
  • Maintain accurate records and uphold professional standards
Requirements:
  • Bachelor of Applied Science (Nursing) or equivalent
  • Registered Nurse, Division 1 with current AHPRA registration
  • Minimum 4 years' nursing experience (acute hospital experience preferred)
