We are seeking a dedicated and compassionate individual to join our team as an Aged & Disability Support professional. As a key member of our community services team, you will play a vital role in enabling our ageing and disabled clients to live a fulfilling life at home.

Key Responsibilities:

  • Deliver high-quality care to clients, following their personalized care plans.
  • Meet the safety needs of clients, using equipment safely and properly.
  • Complete accurate and thorough records and documentation.
  • Conduct nursing assessments to ensure the best possible outcomes for clients.

Requirements:

  • AHPRA registration and a current nursing qualification.
  • Experience in community nursing, subacute care, and disability support.
  • Ability to identify and report client care concerns to the Director of Clinical Operations.
  • Leadership experience and a passion for making a difference in the lives of others.
