You will be responsible for providing high-quality support to our clients, helping them to live their lives to the fullest. You'll work closely with our team to deliver exceptional care and support, making sure that our clients feel valued and respected at all times.
Your key responsibilities will include:
- Meeting with new clients and supporting them through the onboarding process
- Completing comprehensive assessments and developing individualised care plans
- Working with families, health professionals and service providers to deliver quality care
- Monitoring client needs, reviewing progress, and making timely adjustments when required
To succeed in this role, you'll need:
- Experience in case management, aged care, or a related field (typically 3+ years)
- Knowledge of the aged care sector, including the Quality Standards and Code of Conduct
- Strong communication and relationship-building skills
- A client-focused approach, with confidence in advocacy and decision-making
