Are you passionate about making a difference in the lives of our community? We are seeking an ITC Care Coordinator to support our clients.
About This Role:This role involves providing quality care coordination for referred clients with chronic disease and complex needs. The goal is to enable them to access relevant services according to their care plan, aiming for self-management in Primary Health Care (PHC).
The successful candidate will work in the team context, undertaking a range of care coordination activities for Integrated Team Care clients with chronic conditions and complex needs, as outlined in their management plan.
This position will be based in Toowoomba at our programs office.
Key Responsibilities Include:- Working effectively with Indigenous outreach workers to engage with clients.
- Arranging services outlined in the client management plan, such as specialist and allied health appointments, home help arrangements, and connections with support groups.
- Ensuring transport arrangements are in place for clients to attend appointments.
- Involving clients' family or carers appropriately.
- Assisting clients to adhere to treatment programs and develop chronic condition self-management skills.
- Assisting clients to participate in regular reviews by primary care providers.
- Providing clinical care consistent with the requirements of the role, which may involve home or clinic settings.
- Maintaining high-quality client medical records.
- Employing a holistic approach to the client, drawing out cultural or social issues that may impact the client.
- Working with other team members to develop and implement solutions to client needs, including referrals to relevant service providers for support regarding accommodation, finances, well-being, and daily living.
- Building a culturally sensitive health plan that increases client ownership and self-determination, and communicating effectively.
- Enhancing clients' access to urgent and essential allied health or specialist services, necessary transport, and GP-approved medical aids through appropriate use of program funds for eligible ITC clients.
- Experience in providing one-on-one support to clients.
- Ability to collect data, write reports, and maintain client notes.
- Background in working within Aboriginal communities/health/disability services.
- Demonstrated understanding of issues and barriers impacting the safety and well-being of Aboriginal and Torres Strait Islander people.
- Able to interact and communicate with Aboriginal people from diverse backgrounds, showing sensitivity to their culture, history, and current social impacts.
- Effective organizational skills to manage tasks, resources, and deliver outcomes within time and budget constraints.
- Ability to work independently and in a team environment, coping with pressure and meeting tight deadlines.
- Able to interact and engage effectively with administrative staff, medical practitioners, nurses, and other health professionals, as well as external agencies.
- Demonstrated computer skills, particularly in using client information systems.
- Experience working with external health providers and community-based services and organizations.
- Background working with Aboriginal and Torres Strait Islander people, with a clear understanding of their culture.
- Tertiary qualification in community services, health, or another relevant field (preferred but not essential).
- High level of personal resilience and flexibility, with the ability to handle challenging situations.
