Patient Care Specialist

The role involves delivering patient-centered care that coordinates individualized care from admission and throughout hospitalization, incorporating discharge planning in the plan of care.
This includes provision of detailed documentation of a systematic, comprehensive, and accurate assessment of patients, coordinating and implementing intervention(s) identified in the plan of care/clinical path, and evaluating and recording progress towards expected outcomes, revising the plan of care as necessary.
Key responsibilities will include:
- Providing innovative and informed care using contemporary techniques and technology.

- Evaluating and recording progress towards expected outcomes.
Registered Nurses are required to work well within a team environment and promote team cohesion. They must also be able to provide detailed documentation of a systematic, comprehensive, and accurate assessment of patients.
