At HealthX, we are seeking a skilled Registered Nurse to join our team as a Senior Aged Care Clinical Coordinator. Based in Sydney, this is an excellent opportunity for you to work with experienced aged care professionals and make a difference in the lives of our residents.

As a key member of our clinical team, you will be responsible for coordinating the delivery of care to our residents, ensuring that their needs are met and their dignity is respected.

 

The successful candidate will have a minimum of 2 years' experience in aged care, with a strong background in leadership and coordination. You will be passionate about delivering exceptional care and will be able to demonstrate your skills and experience through your application.

 

Key Responsibilities:

  • Coordinate the delivery of care to residents, including medication management and wound care
  • Develop and implement individualised care plans that meet the unique needs of each resident
  • Work collaboratively with the clinical team to ensure seamless transitions of care
  • Monitor and evaluate the quality of care provided, identifying areas for improvement and implementing changes as necessary
  • Provide guidance and support to junior staff members, promoting a culture of excellence and professionalism

 

What We Offer:
