We are seeking a highly skilled Occupational Therapist to join our multidisciplinary team in providing care planning and advance care planning for patients with late-stage chronic and complex health conditions.
This is an exciting opportunity to work in a community-based setting, providing holistic care and support to individuals and families.
Key Responsibilities:- Palliative Care Needs Assessment: Conduct thorough needs assessments and develop collaborative treatment care plans that address the unique needs of each patient.
- Care Planning and Case Management: Provide comprehensive care planning and case management services for community-based patients with chronic and complex illnesses.
- Collaboration and Consultation: Work closely with other clinicians to provide education and support on complex patient needs and contribute to the development of best practice guidelines.
- Registration and Qualifications: Current registration as an Occupational Therapist with AHPRA or eligibility for accreditation as an Accredited Exercise Physiologist with ESSA.
- Experience and Knowledge: Extensive knowledge and experience working with consumers who have chronic and complex illnesses, including ability to conduct advance care planning discussions.
- Clinical Leadership: Demonstrate clinical leadership capabilities, including the ability to provide consultancy, education and support to individuals, families, allied health and nursing staff, and other health professionals.
- Communication and Interpersonal Skills: Possess high level interpersonal and communication skills (written, verbal, computer) and ability to work collaboratively in a multidisciplinary team.
