Care Specialist for Neurological Conditions

We are seeking a dedicated and compassionate care specialist to join our team. As a member of our passionate nursing team, you will provide support to individuals living with neurological conditions.
Key responsibilities include:
- Conducting comprehensive assessments and developing personalized care plans.
- Providing essential nursing services, including continence management, wound care, and palliative support.
- Offering assessment, advisory, educational, and support services to individuals with neurological conditions and their families/carers.
- Coordinating and delegating internal nursing referrals, assessing workload and commitments of each team member.
To be successful in this role, you will require:
- A minimum of 2 years of nursing experience.
- Current registration with the relevant nursing board.
- Current First Aid and CPR certification.
- NDIS Worker Screening Check (or willingness to obtain).
- Full C Class driving license without restrictions.
- Working with children check.
Prior experience in community nursing, along with familiarity with neurological conditions and rehabilitation nursing, is highly desirable.
You will also possess:
- Knowledge of Disability Services Legislation and the National Disability Insurance Scheme.
- Background in palliative care, continence management, and wound care.
- Excellent communication skills, both verbal and written, alongside computer literacy.
- Strong problem-solving abilities and a commitment to quality improvement.
