Case Manager Role

Lives Lived Well is seeking a skilled Case Manager to join our team in Toowoomba. As a key member of our support services, you will work with clients to develop and implement individual treatment goals.

You will be responsible for providing high-quality engagement, assessment, counselling, and support services to clients. This includes managing a caseload, coordinating referrals, and advocating for clients to achieve their goals.

Working closely with dual diagnosis clients requires strong communication skills, empathy, and cultural awareness. You will engage with a diverse population of clients and stakeholders, developing productive relationships to drive positive outcomes.

To succeed in this role, you will need to articulate complex AOD issues and develop solutions that align with evidence-based practice. Your autonomy and adaptability will enable you to thrive in a dynamic environment.

  • Develop and implement individualized treatment plans with clients.
