**Job Opportunity: Community Enabler**

We are seeking a highly motivated individual to join our team as a Community Enabler. This role involves working closely with customers to enable them to thrive in their homes and communities.

**Key Responsibilities:**

The successful candidate will be responsible for:

  • Assessing referrals and onboarding customers who are a good fit for our services
  • Maintaining relationships with key stakeholders
  • Collaborating with colleagues to ensure all work is planned and scheduled effectively
  • Ensuring accurate customer data and facilitating regular customer reviews
  • Managing customer communication channels into and out of the local area

**Required Skills and Qualifications:**

To be successful in this role, you will need:

  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Experience in the in-home and community-based care sector (advantageous but not essential)
  • Willingness to learn and adapt to new situations
