Home Care Nurse Specialist

We are seeking a skilled and compassionate Community Care Coordinator to join our Home Care and Community Nursing Service team.
This is an exciting opportunity for a registered nurse to work closely with clients, their carers, General Practitioners, and other service providers to develop plans that meet individual care needs.
- The successful candidate will have strong communication skills and be able to coordinate and deliver high quality, person-centred nursing care in the community setting.
- They will work collaboratively to develop cohesive and supportive team environments that promote professional growth and development.
About the Role:
- To provide high-quality, person-centred nursing care to clients in their own homes.
- To work closely with clients, their carers, General Practitioners, and other service providers to develop individual care plans.
- To contribute to the development of a cohesive and supportive team environment.
Responsibilities:
- To assess and identify the needs of clients.
- To develop and implement care plans tailored to the individual needs of clients.
- To monitor and review client progress and make necessary adjustments to care plans.
