Job Title: Companion Care Worker

Care Services Overview

Our care services are designed to improve the quality of life for individuals in need.

We provide tailored support for people living with Alzheimer's, dementia, cognitive decline, disabilities, and post-operative care needs.

  • Aged and Disability Care: We offer comprehensive support for seniors and individuals with disabilities, promoting independence and dignity.
  • Transitional Care: Our team provides transitional care services to ensure a smooth transition from hospital to home.
  • Hospital-in-the-Home Care: We deliver high-quality care in the comfort of your own home, reducing the risk of hospital readmission.
  • Palliative Care: Our experienced caregivers provide compassionate support to individuals with terminal illnesses, focusing on comfort and pain management.
Key Responsibilities:
  1. Adhere to Company Policies: You will adhere to our company's policies and procedures, participating in regular Quality Assurance activities.
  2. Homemaking Activities: Perform various homemaking tasks such as vacuuming, dusting, sweeping or mopping floors, doing dishes, changing bed linen, laundry, and cleaning bathrooms.
  3. Meal Preparation: Prepare meals and snacks for clients according to client/family and supervisor's instructions, including grocery shopping when prearranged with the office.
  4. Companionship: Provide companionship for clients, accompanying them on walks, trips to social/recreational activities, assistance with hobbies, etc.
  5. Documentation: Document services provided in the client record and utilize the telephony system effectively.
  6. Communication: Communicate effectively with clients, significant others, and other members of the care team, ensuring confidentiality of client information.
  7. Professional Development: Maintain up-to-date knowledge and skills by attending In-Service training in accordance with identified needs, particularly in relation to Work Health & Safety.
