About the Role

This position will function as a member of a dynamic multi-disciplinary team, supporting the Team Leader and Manager in delivering the Forensic Mental Health in Community Health Program for Aboriginal and Torres Strait Islander participants.

You will be responsible for a range of clinical services, including treatment coordination, intake, mental state examinations, risk assessments, care planning, case management, and discharge planning.

This position upholds Monash Health's purpose and values, with flexibility to adapt to organizational strategic plans.

Key Responsibilities

  • Treatment coordination and case management for Aboriginal and Torres Strait Islander participants
  • Mental state examinations, risk assessments, and care planning
  • Discharge planning and aftercare support
  • Collaboration with multidisciplinary teams to achieve program goals

Requirements
