Job Summary
We are seeking a skilled Registered Nurse/Client Care Coordinator to join our team in the Wide Bay and Bundaberg region. As a key member of our care team, you will be responsible for coordinating client care services, conducting assessments, and providing clinical support to our clients.
Key Responsibilities
- Client Care Coordination
- Meet with clients and their families to assess their needs and develop personalized care plans
- Conduct regular home visits to monitor client progress and adjust care plans as needed
- Clinical Support
- Provide clinical guidance and support to care staff, ensuring high-quality care delivery
- Conduct client care assessments and develop care plans in collaboration with the care team
- Communication and Documentation
- Ensure accurate and comprehensive documentation of client care services and progress
- Communicate effectively with clients, families, and care staff to ensure seamless care delivery
- Compliance and Quality Assurance
- Ensure compliance with relevant laws, regulations, and industry standards
- Participate in quality assurance activities to continuously improve care services
Requirements
- Qualifications
- RN in community nursing with current AHPRA registration
- Or prior Cert III/Cert IV (Aged/Disability Care) with a minimum of 4 years experience
- Skills and Experience
- Experience in aged care, disability, or mental health in a community setting
- Understanding of aged care and disability standards and consumer-directed care
- First Aid, current CPR, and National police check
- WWC (or willingness to obtain)
- Driver's licence and reliable car with comprehensive insurance
- High level of computer literacy
- Understanding of clinical governance and continuous improvement

- Ability to problem-solve and think quickly under pressure
- Ability to multitask with a strong attention to detail while keeping calm and focused
- Excellent networking and consultation skills to develop new referral sources and relationships with external organizations
- Excellent customer service skills with exceptional verbal and written communication skills
- Sound knowledge and understanding of home care services and guidelines
- Knowledgeable in wound care, catheter care, and peg feeding
- Sound knowledge and understanding of NDIS services and guidelines
