Home Care Clinical Support Role

We are transforming home care in Australia through a creative approach that supports people in achieving their goals while living at home. As a key member of our team, you will play a vital role in developing personalized support plans that meet the needs and goals of our clients to live an independent, safe, and fulfilling life at home.

Through individualized case management, you will build meaningful relationships with customers and families, conduct health assessments, and support them to develop a team of carers that fits within their needs and budgets. You will review support plans, mentor, coach, and train non-clinical staff, and provide education on various health topics, such as heart failure, diabetes, or dementia.

Key responsibilities include:

  • Developing and updating support plans collaboratively with customers to achieve their goals
  • Maintaining oversight over customer outcomes and changes in circumstances
  • Providing clinical supervision, assessment, and recommendations for customers accessing our services
  • Acting as a consultant to the team to identify and articulate medical needs and interventions
  • Delivering services in accordance with the Aged Care Act and Aged Care Quality Standards

We seek candidates with:

  • Tertiary qualifications, ideally a Registered Nurse with 5+ years of experience
  • Demonstrated experience in Home and Community Care, ideally with Aged care experience
  • A well-developed understanding and delivery of clinical support and case management experience in Aged care or Disability
  • Current First Aid Certificate
  • Strong communication skills and customer service focus
