Sub-Acute Care Services Leader

We are seeking a skilled and dedicated Sub Acute Care Coordinator to lead patient-centered assessment and discharge planning for individuals admitted to our sub-acute rehabilitation and GEM programs.

Key Responsibilities:- Provide effective communication with patients and families, establish SMART goals tailored to individual needs
- Ensure timely and coordinated discharges
- Manage a care coordination caseload, support clients with complex needs, and liaise with community services and stakeholders to deliver comprehensive care
- Degree in Nursing or relevant field
- Experience in acute care and sub-acute settings
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
