Job Title: Sub-Acute Care Coordinator

We are seeking a highly motivated and experienced Sub-Acute Care Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and coordination services.

  • Key Responsibilities:
  • Lead patient-centered assessment and discharge planning for individuals admitted to our sub-acute rehabilitation and GEM programs.
  • Provide effective communication with patients and families, establish SMART goals tailored to individual needs, and ensure timely and coordinated discharges.
  • Manage a care coordination caseload, support clients with complex needs, and liaise with community services and stakeholders to deliver comprehensive care.
Required Skills and Qualifications:
  • Essential Qualifications:
  • Relevant tertiary qualifications in nursing or a related field.
  • Current registration with the relevant health professional regulatory body.
  • Experience in sub-acute care coordination or a related field.
  • Excellent communication and interpersonal skills.
