Dynamic Occupational Health Specialist

This role involves helping older Australians stay safe, happy and healthy while living in their own homes. You will be responsible for delivering practical advice, services and solutions that enable them to maintain independence and wellbeing.
- Visit clients in their own homes to provide support and guidance that enables them to maintain their autonomy and overall health.
- Deliver a wide range of services including assessments, equipment prescription, home modifications and intervention services, focusing on enhancing physical, functional and cognitive abilities.
- Work with a diverse caseload developing care plans and solutions tailored to each client's individual needs, particularly those with chronic health conditions such as cardiovascular disease, musculoskeletal disorders, diabetes and dementia.
- Collaborate with healthcare professionals to provide high-quality care and support to clients using strong interpersonal skills and effective communication with families, other clinicians and the wider care team.
- Hold a degree or postgraduate qualification in Occupational Therapy.
- Be registered with AHPRA.
- Have a genuine interest in working in the community and with the aged population, with experience being an advantage.
- Be able to work independently and within a multidisciplinary team.
- Possess a current full driver's license.
- Be available for permanent positions.
