Nurse Care Coordinator

The Gurung and Special Care Nursery team at The Sutherland Hospital requires a skilled Registered Nurse to deliver high-quality care to patients and their families.
This opportunity allows you to work in a dynamic environment, coordinating and delivering nursing care that meets the needs of our community.
As a key member of the multidisciplinary team, your role will be responsible for ensuring comprehensive patient care through the coordination and delivery of individualized care plans.
You will need to develop and maintain accurate records of patient progress and outcomes.
Requirements- A current unrestricted registration as a Registered Nurse with the Nursing & Midwifery Board of Australia is required.
- Demonstrated high level written and verbal communication skills, interpersonal skills, and the ability to identify key messages, issues and concerns when communicating with others are necessary.
- The ability to proactively engage with patients/clients to enhance service delivery and guide others in providing service excellence is essential.
- Basic computer skills and proven ability to use health information systems are also required.
- You will be working in a fast-paced hospital environment with a focus on providing exceptional patient care.
