Community Support Worker Job Description

An exciting opportunity exists for a dedicated and innovative professional to join our team and help people with disabilities achieve their goals.

The role involves supporting individuals in various settings, including centre-based programs, community outings, and client homes. You will work collaboratively with clients, families, and other stakeholders to ensure individualized outcomes are achieved.

Key Responsibilities:

  • Assessing the needs of disabled people and planning support programs
  • Interviewing clients and assessing the nature and extent of support requirements
  • Monitoring and reporting on client progress through structured skills assessments
  • Liaising with community groups and agencies about community issues and promoting awareness of resources and services
  • Supporting families and providing education and care for disabled persons in adult service units, group housing, and government institutions

Required Skills and Qualifications:

  • A strong client focus and ability to deliver individualized outcomes
  • Flexibility to work varied shifts, including weekdays, evenings, weekends, and sleepovers
  • Ability to work harmoniously in a team environment
  • Strong problem-solving skills and effective communication skills
  • Demonstrated respect for clients as valued members of their community and understanding of their goals and aspirations
