**Job Title:** Community Health Nurse

**About the Role

This is an exciting opportunity to work with victims/survivors of sexual assault and/or family violence, providing holistic health care services.

You will be part of a dedicated team at the Barwon Multidisciplinary Centre (MDC), collaborating with counsellor advocates and case managers to improve health outcomes for vulnerable individuals.

The MDC CHN service operates from a trauma-informed approach, offering in-reach and outreach nursing services. Your key duties will include:

  • Health assessment and care coordination
  • Providing education and increasing health literacy
  • Assisting clients to engage with health care services

You will work closely with Victoria Police and Child Protection to develop and implement client-centered care plans.

This role requires a skilled and compassionate nurse who can drive initiatives and foster collaboration within the team.
