Job Description:

St Vincent's Virtual and Home programs are expanding to provide more individuals with access to high-quality clinical care at home. These programs are integrated with the hospital and community care network, comprising an experienced, supportive, and dynamic multidisciplinary team that aids people in leaving hospitals sooner, remaining in their homes, and achieving their goals.

The Clinical Lead will be responsible for leading the establishment and management of a centralized Hospital in the Home (HITH) program for private patients. This role entails providing effective leadership to virtual and home care teams, ensuring excellence in clinical practice, patient care, and experience.

Key responsibilities include developing and overseeing processes for patient referrals into HITH and ongoing clinical care coordination, delivering high-quality nursing services within professional boundaries, and contributing to the development and delivery of virtual care solutions nationally.

Required Skills and Qualifications:
  • A minimum of 5 years of postgraduate experience is required.
  • At least 2 years of nursing leadership experience in acute, emergency, or community settings is necessary.
  • Current nursing registration with AHPRA and a valid driver's license are mandatory.
