We are seeking a skilled Case Manager to work with vulnerable children and young people in Out Of Home Care (OOHC). The role involves case managing clients, supporting their carers and birth families, and ensuring the best outcome for the child.
About the Position:- The successful candidate will be responsible for working with designated clients, including children, young people, and their families.
- The Case Manager will engage with clients, using appropriate assessment and risk mitigation strategies.
- Key responsibilities include conducting thorough assessments to develop, implement, and review client plans, ensuring active involvement of children and young people.
- Transparent communication is essential by openly sharing client plans with clients and relevant agencies.
- The Case Manager will coordinate with various service providers to ensure comprehensive support delivery.
