Care Coordinator Specialist

This role involves working with young people transitioning from care to adulthood, providing outreach support and developing tailored plans for their transition.

About the Role:
The Transition Case Manager will create comprehensive case plans responding to individual needs of children and young people in our care.
Key Responsibilities:
- Develop detailed plans for each young person to support their transition to independent living.
- Offer guidance, advice and referrals to individuals who have left our care up to the age of 25.
- Collaborate with Cultural Support Planners to implement strategies within case plans for Aboriginal and Torres Strait Islander children and young people.
- Drive the implementation and achievement of case plan goals and permanency plans for children and young people.
Requirements:
- A relevant bachelor's degree in Social Work, Social Welfare, Psychology, Nursing or Mental Health.
- A current driver's licence.
- Demonstrated experience in case planning with children and young people who have experienced trauma.
