Job Description:

The ideal candidate for this Healthcare Professional role will be a motivated and experienced doctor who is passionate about delivering exceptional patient care. They will have a strong focus on Chronic Disease Management (CDM) and a willingness to work collaboratively as part of a multidisciplinary team.

Key Responsibilities:

  • Provide primary medical care to patients of all ages, including chronic disease management and health promotion
  • Conduct thorough consultations, taking a holistic approach to patient care
  • Develop and implement individualized care plans for patients with complex needs
  • Collaborate effectively with other healthcare professionals, including nurses and allied health staff, to ensure comprehensive patient care
  • Participate in regular team meetings and contribute to the development of practice policies and procedures
  • Stay up-to-date with the latest medical research and guidelines, incorporating evidence-based practice into daily care

Practice Details:

  • Located in a busy rural town with a small primary healthcare offering
  • High volume of CDM patients, driving an integrative care focus
  • Accredited practice with a strong existing patient base
  • Supportive administrative staff, providing excellent support services
  • Pharmacy and other destination health services nearby

Working Conditions:

  • Monday to Friday office hours only
  • Fully equipped treatment room and procedural nurse support
  • Care plan nursing support Monday to Friday
