Telehealth Care Specialist Opportunity

We are seeking a skilled Telehealth Care Specialist to join our team, providing high-quality, co-managed clinical care to aged care residents.

This unique opportunity combines telehealth and face-to-face services, offering flexibility and the chance to make a meaningful impact.

The ideal candidate will have current AHPRA registration as a General Practitioner, a passion for aged care, and comfort working independently via telehealth.

  • Conduct remote telehealth sessions with on-site nurse support
  • Provide after-hours or weekend coverage via telehealth
  • Collaborate with nursing and geriatrician teams
Key Responsibilities:
  • Support aged care residents with comprehensive assessments and reviews
  • Work across multiple service streams
  • Provide in-person aged care visits, including admissions, reviews and care planning
Requirements

FRACGP or FACRRM

Comfortable working independently via telehealth

Passion for aged care and improving access for vulnerable populations

Current AHPRA registration as a General Practitioner
