**Job Title:** Care Manager

Job Description:

As a dedicated Case Manager within the Home Care Packages department, you will develop care plans with consumers to meet their individual goals and coordinate the delivery of services. You will provide support, direction, and training to the Home Care team.

Key Responsibilities:

  • Ensure comprehensive assessments of consumer care needs and risks
    - Develop goal-directed care plans that identify required supports and services to achieve goals while minimizing and managing risk.
    - Lead the Home Care Team to deliver high-quality and compliant services.
  • Report to the Home Care Operation Manager on program performance against key performance indicators (KPIs) and budget performance.
  • Maintain a collaborative and effective relationship with all key stakeholders.

Required Skills and Qualifications:

- Degree in Social Work, Social Sciences, Nursing, or equivalent qualifications.

- 3+ years' experience as a case manager providing consumer-directed support and/or services.

- 5+ years' experience in leadership/management roles in the Aged or Disability industry.

Benefits:

$90,000 - $91,000 per year
