The successful applicant will work collaboratively with a multidisciplinary team to deliver assessment, care coordination, and education to families, taking into account clinical, psychosocial, and cultural needs.
Key responsibilities include:
- Providing high-quality nursing care to Aboriginal families, working independently and collaboratively as part of a dynamic team.
- Conducting home visits and offering practical health support based on primary health care principles, ensuring effective engagement with families and healthcare teams.
- Collaborating with Aboriginal Health Workers and allied health professionals to develop personalized care plans, promoting early intervention and prevention strategies.
- Educating families about early childhood development, health promotion, and parenting strategies, advocating for Aboriginal health needs and contributing to continuous service improvement.

What You'll Be DoingAs an Aboriginal Health Advocate, you will be responsible for delivering culturally responsive health services to Aboriginal families, working collaboratively with a multidisciplinary team to provide assessment, care coordination, and education.
Where You'll Be WorkingSouth Western Sydney Local Health District provides a comprehensive range of community-based services, including prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The district supports diverse populations, offering specialized programs for children, young people, older adults, and at-risk communities.
Required Skills and Qualifications- Certificate III in Aboriginal & Torres Strait Islander Primary Health Care or relevant qualification.
- Demonstrated experience delivering culturally responsive health services to Aboriginal communities.
- Strong communication and interpersonal skills, ensuring effective engagement with families and healthcare teams.
- Ability to work independently and collaboratively in a dynamic, client-centered environment.
- Commitment to continuous learning, professional development, and culturally safe practices.
This is an Identified Aboriginal/Torres Strait Islander Position. Applicants must be of Aboriginal descent through parentage, identification as being Aboriginal, and accepted in the community as such. Exemption is claimed under Section 14 of the Anti-Discrimination Act 1977.
