
Care Coordinator Position

The Support Coordinator role is a trusted position at an organisation that delivers personalised, culturally sensitive, and person-centred care to individuals with disabilities.
- This role involves working closely with participants to help them understand and implement their NDIS plans.
Key responsibilities include:
- Connecting participants with appropriate services, providers, and community resources.
- Building capacity in participants to exercise choice and control in achieving their goals.
- Developing and maintaining strong professional relationships with families, stakeholders, and service providers.
- Monitoring progress, addressing barriers, and ensuring services are delivered in line with participant needs.
To be successful in this role, you will need:
- Previous experience as a Support Coordinator or in a similar NDIS role.
- A strong understanding of the NDIS framework and funding streams.
- Excellent communication and problem-solving skills.
- The ability to work independently and as part of a supportive team.
- An appreciation for cultural diversity and lived experiences.

Pay: $33.00 – $40.00 per hour
