
Clinical Case Manager

A full-time position has become available as a Case Manager within our department, based in Warrnambool. This role involves working with individuals at medium or high risk of reoffending, utilizing evidence-based case management practices to achieve positive outcomes.
Some travel may be required across the South West region, with a focus on applying case management principles to complex individuals and promoting rehabilitation and public safety.
Key Responsibilities- Identify and assess factors related to individual offending behaviour
- Motivate and influence individuals in the case management process
- Produce detailed reports and other standard documents in line with procedural guidelines
- Monitor individuals' compliance with order requirements
- Exhibits assertiveness and confidence when dealing with challenging behaviours
- Develops positive working relationships and works effectively in a close-knit team environment
- Understands and applies case management principles to support complex individuals
- Models pro-social behaviours and motivates others to adopt similar attitudes
- Recognizes the importance of cultural awareness and promotes an inclusive culture
As a Case Manager, you will play a critical role in reducing reoffending risk and engaging individuals to provide targeted interventions. This position supports rehabilitation and public safety by applying evidence-based case management practices.
TravelSome travel may be required across the South West region, dependent on the needs of the role and the department's operational requirements.
Qualifications and Requirements- Ability to apply case management practices to support individuals with complex needs
- Strong written and verbal communication skills for report writing and documentation
- Eligibility for pre-employment checks (police checks, reference checks, and other assessments as required)
