Overview

A full-time position has become available as a Case Manager within our department, based in Warrnambool. This role involves working with individuals at medium or high risk of reoffending, utilizing evidence-based case management practices to achieve positive outcomes.

Some travel may be required across the South West region, with a focus on applying case management principles to complex individuals and promoting rehabilitation and public safety.

Key Responsibilities
  • Identify and assess factors related to individual offending behaviour
  • Motivate and influence individuals in the case management process
  • Produce detailed reports and other standard documents in line with procedural guidelines
  • Monitor individuals' compliance with order requirements

  • Develops positive working relationships and works effectively in a close-knit team environment
  • Understands and applies case management principles to support complex individuals

About the Role

As a Case Manager, you will play a critical role in reducing reoffending risk and engaging individuals to provide targeted interventions. This position supports rehabilitation and public safety by applying evidence-based case management practices.

Travel

Some travel may be required across the South West region, dependent on the needs of the role and the department's operational requirements.

Qualifications and Requirements
  • Ability to apply case management practices to support individuals with complex needs
  • Strong written and verbal communication skills for report writing and documentation
  • Eligibility for pre-employment checks (police checks, reference checks, and other assessments as required)
