Care Coordinator for Vulnerable Children

We are seeking a skilled and compassionate Therapeutic Case Manager to support children and young people in foster care. Your expertise will play a vital role in delivering high-quality care, ensuring the best outcomes for our clients.
About the RoleThis is an ongoing part-time position, working 4 days a week. You will have the opportunity to make a positive impact on the lives of vulnerable children and young people.
As a Therapeutic Case Manager, you will be responsible for coordinating caregiver matches, facilitating transitions, and providing specialized care to ensure each child receives the support they need to overcome their past experiences and achieve their full potential.
Our team works collaboratively with foster carers, families, and a network of professionals to provide comprehensive support to our clients.
Key Responsibilities- Facilitate transitions and coordinate caregiver matches to ensure the best possible outcomes for children and young people
- Provide specialized care and support to help children and young people overcome their past experiences and achieve their full potential
- Work closely with foster carers, families, and a network of professionals to provide comprehensive support to our clients
- Ensure safe environments for children and young people and protect them from abuse and neglect
- Post-secondary qualifications in Welfare, Social Work, Child Development or relevant qualification (if Diploma level, willing to study further)
- Current Victorian drivers licence
- Current CPR and First Aid certificate
- Demonstrated ability to communicate effectively with adults, adolescents, children and sector representatives
- Salary: SCHADS Level 5 + super
