Key Responsibilities:- Conduct comprehensive assessments of clients' home support needs using the Integrated Assessment Tool (IAT).
- Determine eligibility for home support services and develop tailored support plans to meet clients' evolving needs.
- Collaborate with multidisciplinary teams, including clients, families, and healthcare professionals, to ensure seamless care delivery.
- Maintain currency of knowledge on aged care services, both funded and non-funded, to inform assessment decisions.
- Tertiary qualification in a health-related discipline directly related to health, aged care, or a specialist area (e.g., Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist, or Social Worker).
- Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
- Australian Driver's license and access to a registered, insured vehicle.
- Minimum one year demonstrated experience delivering services in aged care settings and/or to older adults, including those living with dementia.
- Flexible work arrangements to balance work-life demands.
