
Compassionate Community Care Nurses Wanted

We are looking for skilled nurses to provide clinical in-home services and support our community. Our ideal candidate will have a passion for delivering high-quality care, excellent communication skills, and the ability to work independently and as part of a team.
Key responsibilities include:
- Providing clinical in-home services including care planning, medication management, wound care, chronic disease management, palliative care, and diabetes management.
- Supervising non-clinical team members in the community.
- Handling intake, care planning, and reporting.
- Conducting client assessments with a focus on Consumer Directed Care.
- Coordinating quality, person-centered care tailored to client needs and funding.
- Managing documentation in compliance with regulations.

About the Role:
- This is a casual role working with seniors, adults with disabilities, and those recovering from hospital stays in Mandurah, Falcon, Greenfields areas.
- Provide complex medical support and care planning.
- Residing in Western Australia is required.
- Qualified Enrolled Nurse
- Current AHPRA registration
- First Aid & CPR certification
- Australian Police Check
- Professional Indemnity Insurance
- Experience in community care assessment and planning
- Valid driver's license and reliable vehicle
- Smart mobile device (iPhone/Android)
- Passion for high-quality, client-focused community care
- Cert IV in Training and Assessment (highly desirable)
- Strong communication skills
- Ability to work independently and in teams
- Excellent time management and organizational skills
- Commitment to ongoing professional development
- Residing in Western Australia is required
