Job Title: Community Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team in delivering exceptional care to clients in their own homes.

The successful candidate will have the ability to work autonomously and as part of a team, with a strong focus on patient-centered care. They will be responsible for conducting client assessments, developing and implementing care plans, and providing high-quality clinical care.

Key Responsibilities
  • Clinical in-home services including care plan assessment, medication management, incontinence care, wound care, chronic disease management, complex wound management, palliative care, health and diabetes management
  • Clinical delivery and supervision of non-clinical team members in a community setting
  • Intake processing, clinical care planning and reporting
  • Conducting client care assessments with a focus on Consumer Directed Care
  • Coordinate the provision of quality person-centred care according to the client's needs, service/care plans and available funding
  • General administration for documentation relating to client care and in alignment with AHPRA regulations and legislation
Qualifications and Requirements
  • Qualified Registered Nurse
  • Current AHPRA registration
  • First Aid & CPR certification
  • Australian National Police check
  • Professional Indemnity Insurance
  • Experience in assessing and planning care needs within the community
  • Valid Driver's Licence and reliable, presentable car
  • A smart mobile phone (iPhone/Android)
Preferred Skills
  • A passion for high-quality client-focused care in a community setting
  • Certificate IV in Training and Assessment - highly desirable
  • Exceptional communication skills
  • Able to work autonomously or within a team environment
  • Excellent time management and organisation skills
