Job Overview:

We are seeking a passionate wellbeing professional to lead our care services on a full-time, 12-month contract and deliver high-quality support across the Queensland region.

A Day in the Life:

  • Coordinate personalized care plans and manage delivery, budgets, and clinical support to ensure holistic, person-centred care at home.
  • Provide nursing care one day per week, including assessments, treatments, and care planning as part of the wellbeing manager role.

Key Responsibilities:

  • MUST HAVE Current Registered Nurse (AHPRA) registration with hands-on experience and a passion for supporting older adults to live independently.
  • Strong case management skills (ideally in community/aged care), HCP assessment experience, and a goal-oriented, holistic approach.
  • Excellent communication and stakeholder engagement skills, valid driver's licence, and willingness to travel locally.
