About the Role

The Aged Care Support Specialist plays a vital role in ensuring continuity and consistency in the delivery of person-centred care to participants in the Community Care program.

Key Responsibilities
  • Act as the primary point of contact for participants, identifying their needs, goals, personal preferences, and existing supports, providing timely information on available services.
  • Communicate with relevant stakeholders involved in the delivery of services regarding participants' needs and wellbeing to action the care plan and associated recommendations in a timely manner.
  • Work collaboratively within a team-based model of care, escalating clinical decisions appropriately and participating in shared planning and problem solving to improve participants' health outcomes.
  • Support participants in making informed decisions, respecting their right to take risks and engaging with registered supporters and carers as appropriate.
  • Assist participants, their registered supporters, and family members in navigating age-related systems and programs, linking them to additional support in line with their care plan needs and goals.
Requirements

To be successful in this role, applicants should possess:

  • Tertiary qualification in Health or a related field.
  • Diploma-level qualification in Community Services, Aged Care, Case Management, or equivalent experience.
  • Formal training in trauma-informed care, cultural safety, or restorative practice.
  • Strong background in aged care - home care, care management, or a healthcare setting.
  • Sound understanding of compliance in Aged Care and NDIS, including quality standards and Work, Health and Safety (WHS) regulations.
  • Proficiency in using care management or Customer Relationship Management (CRM) systems.
