Advanced Nurse Practitioner Job Opportunity

Job Summary

We are seeking an experienced Advanced Nurse Practitioner to join our team in driving healthcare plans for local communities. As a key member of our team, you will be responsible for delivering high-quality patient care and developing strategies to reduce hospital access.

Key Responsibilities

  1. Triage patients via telephone and assess the need for face-to-face appointments.
  2. Work as an autonomous clinician, taking responsibility for assessment, treatment planning, and referrals to specialist services when required.
  3. Comply with professional conduct guidelines and abide by national regulations.
  4. Take comprehensive patient histories, perform physical examinations, and use expert knowledge to identify potential diagnoses.
  5. Refer patients for investigations and/or specialist services where necessary.
  6. Independently decide on and carry out treatment, including prescribing medications.
  7. Make onward referrals to other services as appropriate.
  8. Ensure continuity of care.
  9. Work independently and collaboratively within a local and wider healthcare team.
  10. Provide leadership to the team and ensure each patient's treatment is based on best practice.

Requirements

  • Proven experience in delivering high-quality care within Primary Care settings.
  • Ability to handle a wide range of minor ailments.
