Job Opportunity:

We are seeking a dedicated and compassionate Healthcare Advocate to join our team.

The ideal candidate will possess excellent communication skills, be passionate about promoting preventative health, and have experience in Chronic Disease Management and health assessments.

This role involves advocating for the local community, supporting GPs and patients in managing chronic conditions, and collaborating with multidisciplinary care teams.

The successful candidate will have:

  1. AHPRA registration as an Enrolled or Registered Nurse
  2. Experience in Chronic Disease Management and health assessments
  3. Knowledge of Health Assessment (highly regarded)
  4. Strong organizational and time management skills
  5. Excellent communication and customer service skills
  6. A personable approach and passion for people

Responsibilities include:

  1. Prepare Care Plans and Team Care Arrangements for patient disease management
  2. Empower patients to take ownership of their health
  3. Coach patients on health behavior change principles
  4. Facilitate access to ongoing multidisciplinary care
  5. Conduct health assessments for early identification of chronic disease
  6. Collaborate with GPs to promote preventative healthcare
  7. Increase the number of patients receiving initial care plans
  8. Review patient care plans regularly
  9. Manage administrative tasks related to CDM appointments
