Role Summary:

We are seeking an experienced Clinical Assessor to join our team based on the Central Coast. This 6 month Fixed Term position offers flexibility and autonomy, with opportunities for local travel within the region.

This role is responsible for conducting assessments to identify and meet the complex needs of clients and referring to appropriate aged care services under the Aged Care Act.

The successful candidate will be a skilled professional with strong communication and interpersonal skills, able to work collaboratively with clients, families, and multidisciplinary teams.

Key responsibilities include conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services.

The role also involves reviewing support plans to meet client's changing support needs.

Key Responsibilities:
  • Conduct assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care Act
  • Collaborate with clients, families, and multidisciplinary teams as required
  • Work collaboratively with colleagues to promote a supportive and positive team culture
  • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services
  • Review support plans to meet client's changing support needs
Requirements:
  • Tertiary qualification in a health-related discipline directly related to Health, Aged Care or related specialist area
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
  • An Australian Driver's license and access to a registered, insured vehicle
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia
