We are seeking a skilled professional to join our team in delivering coordination services to NDIS participants across various age groups. This position offers a permanent role with a flexible work arrangement, allowing for both remote work and collaboration at our office. You will receive ongoing support and have easy access to management and team members, with some travel required for work-related purposes.
In this role, you will assist participants in understanding and executing their plans, ensuring alignment with their personal goals, fostering capacity-building, and enhancing resilience. You will also connect participants with tailored informal, formal, and mainstream supports.
Key Responsibilities:
- Provide expert guidance and support to participants, including discussions on NDIS plan goals and objectives.
- Collaborate with families and participants to enhance their ability to independently manage their supports, which may involve:
- Identifying suitable services and supports.
- Offering assistance and mentorship to resolve service delivery challenges.
- Supporting participants and their families to ensure budget alignment with service usage.
- Monitor and report on participant outcomes, coordinate services, and operate within the financial limits of each plan.
- Develop and maintain strong local networks and knowledge of service providers.
- Perform other duties as required.
- Understanding of disability services, NDIS, and inclusion principles, with the ability to quickly acquire knowledge of mainstream and community services.
- Capacity to identify barriers to support and navigate complex family dynamics to achieve participant goals.
- Solid comprehension of the impact of disability on individuals, families, and stakeholders.
- Strong communication and interpersonal skills, including conflict management and resolution.
- Able to manage a busy caseload, meet deadlines, and prioritize effectively.
- Ability to represent the organization positively and build strong community relationships to maximize resources and enhance outcomes.
- Capability to work independently while also contributing to the support coordination team.
- Proficient in technology, including client information management systems and Microsoft Office.
- Ability to collaborate effectively with team members and external stakeholders to ensure seamless service delivery.
- Relevant qualifications and/or equivalent industry experience.
- Possession of a Blue Card, NDIS Worker Screening Card, and a suitable Police Certificate, or the ability to obtain these.
- Current Australian Driver's License.
- Commitment to the organization's values and philosophy.
